Grant Report Form

When your project is finished, a final report must be completed.  Time frame for completing the project is
approximately one year from receipt of funds.

• How the funds were used.

• How the project served the Prospect Park neighborhood.

• Numbers of people served including number of community residents.

• Please include several pictures of your project

Note:

If you received funds in the prior year, a final report must be received before new funding will be considered. 
It can be included with the grant application but we would appreciate receiving it before a new application.
